Dear Parents,

Attached are the registration forms for the Queen of Peace Preschool. Registration forms

will be accepted beginning January 27. A commitment deadline of February 8 is required for

our current students. Acceptance letters will be sent February 15, 2008. When the classes

are filled, names will be put on a waiting list.

Please return the registration packet along with the $50.00 registration fee when

registering. The registration fee is non-refundable.

The general information medical form and physician’s form are due by the first day

of school. A parent and a physician must complete these forms. State law requires that a

preschooler must have a physical examination within twelve months prior to the first day of

attendance at school and annually from the date of the examination thereafter. A copy of your

child’s birth certificate should also be submitted by the first day of school.

Tuition for the 2007-2008 year was $110.00 per month for the two day class and

$130.00 per month for the three day classes. This amount is subject to change for the

2008-2009 school year. Two day classes are Thursday and Friday mornings. The three day

classes meet on Monday, Tuesday, and Wednesday mornings and afternoons. Morning

classes are 9:00 a.m. – 11:20 a.m. and the afternoon classes meet 12:00 – 2:20 p.m.
